  • Minimum of two (2) years experience directly supervising Supports Planners and/or Coordinators of Community Services (CCS).

  • BA or BS from four-year college or university in Psychology, Social Work, Nursing, Counseling, Human Services, preferred or a related field with work pertaining to adults with chronic conditions and disabilities.?(Candidates with extensive case management and supervision experience without a degree may be considered if currently enrolled in a Bachelor’s program.)

  • Minimum three years of case management experience, preferably working with people with complex medical and/or behavioral health needs, older adults and/or adults, children, and youth with disabilities.

  • Experience with coordination with other organizations and programs that serve individuals with complex medical and/or behavioral health needs, older adults, and individuals 16 years and older with disabilities in community-based settings preferred.

  • Excellent communication skills-writing and speaking.?

  • Computer skills including Word, Excel, and PowerPoint.?

  • Creative problem solving and flexibility.?

  • Training and mentoring experience.?

  • Ability to manage complicated work processes and multi-task.?

What You Will Do:The Tea

m Manager, Medical Day supervises a team of Case Managers providing services to applicants/participants (clients) in the Medical Day Care Services Waiver (MDCSW) Program. The Team Manager may carry a caseload and supports the mission of The Coordinating Center by providing person-centered service coordination within the parameters of the MDCSW contract. Team Managers are responsible for monitoring compliance with specific contract requirements, maintaining data and providing technical and analytical support in contract planning, data extraction, report writing and maintenance of all documentation. This position reports to the Program Director, Medical Day and works interdependently with The Center’s MDCSW team to maximize quality outcomes for individuals receiving MDCSW services.


As the Team Manager you will:

  • Assume day-to-day coordination of the MDCSW Program functions and provides direct supervision to team members.??

  • Serves as a resource to staff for problem solving regarding client issues, contract requirements, policies, and procedures, eligibility; advises, supports, and takes appropriate action.?

  • Coordinate and manage external and internal communications and resources to maintain quality service delivery.?

  • Facilitate effective teamwork and maintains an efficient and respectful work environment.?Conducts regularly scheduled team meetings.?

  • Act as first point of contact with Case Managers and the Maryland Department of Health (MDH) regarding problems concerning care management issues, contract requirements, policies and procedures and other issues.?Fields questions to Program Director as appropriate.?

  • Monitor Case Managers work and documentation to ensure that clients are receiving quality care management.?

  • Assist the Director with the development of policies, procedures, processes, and day-to-day operations as appropriate.?

  • Facilitate change management through communication and assisting the Director with ensuring the policies, procedures and processes are up to date and accurate.?

  • Ensure timely completion of all performance evaluations and coaching and counseling sessions.?Troubleshoots staffing issues with the Director and takes appropriate action.?

  • Assure that clients assigned to team have an assigned Case Managers and/or have a point of contact when the Case Manager is not available, ensuring the team is appropriately covered.?

  • Maintain a client load and delivers case management services per The Center’s protocols adhering to the contract requirements.?

  • Attend meetings, as appropriate, in Director’s absence.?

  • Communicate with other Team Managers to provide consistency across teams and programs.?

  • Interview, orients, trains, and mentors newly hired staff on MDCSW concepts, service delivery and contract specific processes, tools, and procedures.?

  • Perform daily review of staff work to ensure billing compliance, provide feedback regarding the completeness, timeliness, and quality of documentation.?

  • Continually assesses the resources, learning opportunities, and training needed by staff.? Communicates those needs to the Director for discussion and implementation of the course of action.?

  • Manage multiple priorities and tasks with flexibility to enhance service coordination.?

  • Maintain reports as contracts require ensuring accuracy and following up with Case Managers as needed. Assist with periodic contract audits.?

  • Collaborate with Human Resources (HR) and Director to promote compliance with HR policies.?

  • Lead by example and implements The Center’s mission/values within the department.

The salary for this position is $68,000 with the opportunity for quarterly success payments and mileage reimbursement.
