Job Description –Psychiatric Mental Health Nurse Practitioner
Position Summary:
Provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders and health maintenance to adults. This includes the assessment, diagnosis, and management of mental health problems and psychiatric disorders. The NP works collaboratively with other on site medical and mental health providers as well as with community PCPs.
The clinic utilizes Kareo as its EMR system.
Supervision – Report directly to Administrator/Clinical Manager. Medical oversight provided through practice agreement with collaborating Physician/Psychiatrist.
Typical Duties & Responsibilities:
50% Patient Care Management/Documentation
- Work collaboratively with a physician and/or mental health provider of record, (if one exists), to provide comprehensive mental health services to patients.
- Through patient evaluation, determine a differential diagnosis and plan for intervention.
- Adhere to standards via identified text and/or via office practice standard protocols.
- Maintain confidentiality and patient privacy.
- Document care provided; meeting standards for quarterly chart reviews.
20% Consultation and Referral
- Seek consultations as appropriate, act as a consultant as requested.
- Facilitate the process of ethical decision making as appropriate.
- Perform comprehensive assessments and refer patients for further care and services when necessary.
15% Patient Education
- Utilize teaching learning theory with patients and their families to influence patient outcomes.
- Identify learning needs of various populations and contribute to the development of educational programs to meet those needs.
15% Communication/Citizenship
- Identify system specific problems, which require evaluation and/or change; work with appropriate staff to resolve these issues by participating in performance improvement processes.
- Takes an active role to contribute to the improvement of office functioning.
Skills Required:
Ability to communicate clearly both in writing and verbally with patients and healthcare team members.
Ability to function independently with regards to most clinical decision making.
Ability to relate well to others. Well versed in mental health and assessment.
Education/experienced required:
Masters prepared Psychiatric Nurse Practitioner/PMHNp/APRN
