Job Summary
The HIV Patient Liaison serves as an advocate for CORE patients serving as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within CCH and in the community. Gathers the required documents for Ryan White eligible patients, including payor of last resort, and validates that meet the requirements, as per funder (CDPH and HRSA). Conducts barriers assessments to ensure patients are linked and engaged in care through facilitating patient appointments, including those made with labs, diagnostic areas, and specialty physicians. The HIV Patient Liaison is responsible for providing daily oversight and optimization of registration and throughput of patients from front desk to providers of care. The HIV Patient Liaison will be responsible for clerical staffing support, registration integrity, resource management, quality assurance, denial report management, mitigation of erroneous charges and customer service throughout the ambulatory clinics. This position is grant funded until February 28,2026.
Typical Duties
- Serve as an essential link between patients and care providers.
- Gathers and scans the required documents for Ryan White eligible patients, including payor of last resort, and validates that meet the requirements, as per funder (CDPH and HRSA).
- Initiates greetings and positive interaction with patients, visitors, and staff
- Answers all calls and directs them to appropriate department.
- Represents the CCH mission, vision, and values by acting in a courteous, respectful, attentive and service oriented manner with patients.
- Schedules patient appointments, opens and closes providers schedules, reschedules make-up appointments.
- Provides registration and scheduling support to facilitate continued operations.
- Ensures documentation is accurate, current, and aligned with standard operating procedures.
- Makes appointment reminder calls and updates system with patient insurance.
- Reviews and responds to patient portal messages, voicemails, incoming calls for appointments, and connections to other multidisciplinary team members where appropriate.
- Coordinates the data collection and storage of confidential patient information for reporting purposes.
- Complies with CCH policies and procedures to ensure high quality service delivery.
- Assists patients in accessing HIV care and navigating healthcare systems through completing referrals in a timely manner.
- Provides support and coverage within the department, as needed.
- Assists in report preparation and submission.
- Sorts, handles, and distributes office correspondence.
- Troubleshoots equipment malfunctions, calls for repairs, and evaluates new equipment providing feedback to management.
- Adheres to HIPAA standards and complies with patient confidentiality policies for the retention of patient information, handling, distribution, or disposal of patient health information.
- Attends and participates in meetings, including multidisciplinary committee meetings as required.
- Performs other duties as assigned.
Minimum Qualifications
- High school diploma or GED equivalent is required
- Two (2) years of database experience working in a healthcare or clinical setting is required
- Experience working with patients impacted with chronic conditions related to HIV/AIDS and other infectious diseases, gender-affirming care individuals, persons with substance abuse, mental health, and/or homeless issues is required
- Proficient computer skills including Word, Excel, Access and Power Point is required
Preferred Qualifications
- Bachelor’s degree from an accredited college or university is preferred (Official transcripts are due at the time of interview)
- One (1) year of experience working with HIV patients and programs is preferred
- Experience and knowledge of medical and medication terminology is preferred
- Bilingual is preferred
