What are the responsibilities and job description for the Care Navigator position at Conway Medical Center?
Position Summary:
The Care Navigator (CN) will provide psychosocial assessments, diagnosis, and treatment, as well as discharge planning to and consultation about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders.
Qualifications:
Education:

Bachelor’s degree in social work (BSW) required.

Master's degree (MSW) from an accredited school of social work preferred.

Licensed Practical Nurse (LPN).
Experience
-
Minimum of three years of social work experience required.

Previous social work experience in health care setting/hospital preferred.
Licensure/Certification/Registration
-
Licensed Social Worker or Licensed Practical Nurse.
Duties & Responsibilities:
-
Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions.

Interventions may include crisis intervention, brief and long-term individual, marital, family and/or group therapies.

Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.

Arrange, procure, and coordinate patient/family pre and post hospital needs.

Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child, and elder abuse, etc.

The CN will complete other duties as assigned.

Greet all patients and their accompanying family members when applicable and provide exemplary customer service.

Work effectively and collaboratively with colleagues, physicians, and department heads.

Effectively utilize strong organizational skills.

Consistently display effective verbal and written communication skills.

Proficient understanding and use of technology/PC skills required.
