Position Summary:
The Admission RN (AN) will collaborate with interdisciplinary teams to coordinate resources and ensure that patient admissions meet established standards of care. The AN is responsible for gathering patient admission history, facilitating a smooth transition, and promoting quality outcomes. The Admission Nurse will be expected to provide direct patient care to an assigned unit as determined by the AVP or the CNO
Qualifications:
Education:
- Associate degree (ASN) in Nursing required.
- Bachelors’ degree (BSN) in Nursing preferred.
Experience
- Three (3) years acute care experience preferred.
- Knowledge of nursing theory and practice required.
Licensure/Certification/Registration
- Current South Carolina (or compact state) licensure as RN (SCLLR) in good standing required.
- Basic Life Support (BLS) certification required.
- ACLS certification preferred.
Duties & Responsibilities:
- Ensure Completion of Admission Requirements: The AN would verify that all necessary admission requirements are completed.
- Medication History: They would ensure accurate medication list, to include confirming patient preferred pharmacy.
- Initiation of Admission Order Sets: The AN would be responsible for ensuring that all admission order sets are initiated.
- Initiation of Plan of Care: The AN would ensure initiation of plan of care based on admission diagnosis and patient assessment.
- Patient and Caregiver Education: The AN would provide welcome packet and admission education.
- Transporting: AN would assist with transporting patients to inpatient units, effectively completing the nursing hand-off process.
Standards of Performance:
- Monitoring and Quality Metrics: The AN would identify and recommend quality and process improvement initiatives and assists with the development and execution of action plans.
