Contract Costa Health Plan (CCHP) Case Management team is looking to fill one (1) vacant Health Plan Outreach Representative (HPOR) position! This position will be located in Martinez.
Our Case Management team works closely with Medi-Cal, (future) Medicare, and Commercial members to assess their needs, coordinate care, and connect them with medical, behavioral health, and social support services. By addressing complex health and social needs, we help members achieve better outcomes and live healthier, more independent lives.
Through the CalAIM Department, which oversees Enhanced Care Management (ECM) and Community Supports, our team helps remove barriers to care such as transportation, housing, nutrition, and other social determinants of health. We collaborate with providers, community organizations, and other health plan teams to ensure members receive holistic, person-centered support that addresses both medical and non-medical needs.
In this role, you’ll have the opportunity to make a tangible impact by guiding members through the health care system, improving access to services, and helping them navigate complex programs. You’ll gain experience in innovative care delivery, work alongside passionate professionals, and contribute to programs that advance health equity across Contra Costa County. Minimum Qualifications
License Required: Possession of a Valid California Motor Vehicle Operator's License
Education: Possession of a high school diploma, G.E.D. equivalency or California High School Proficiency Certificate.
Experience: One (1) year of full-time, or its equivalent, experience performing marketing, sales, promotions, customer service or field service activities in a managed care program working with low income populations.

CONVICTION HISTORY
