Contract Costa Health Plan (CCHP) Case Management team is looking to fill one (1) vacant Health Plan Outreach Representative (HPOR) position! This position will be located in Martinez.

Our Case Management team works closely with Medi-Cal, (future) Medicare, and Commercial members to assess their needs, coordinate care, and connect them with medical, behavioral health, and social support services. By addressing complex health and social needs, we help members achieve better outcomes and live healthier, more independent lives.

Through the CalAIM Department, which oversees Enhanced Care Management (ECM) and Community Supports, our team helps remove barriers to care such as transportation, housing, nutrition, and other social determinants of health. We collaborate with providers, community organizations, and other health plan teams to ensure members receive holistic, person-centered support that addresses both medical and non-medical needs.

In this role, you’ll have the opportunity to make a tangible impact by guiding members through the health care system, improving access to services, and helping them navigate complex programs.   • Educating members about their benefits and Case Management/CalAIM programs.
  • Welcoming new members and connecting them with services and resources.
  • Helping members navigate medical, behavioral health, and social support services.
  • Assisting with member questions, concerns, and complaints in a professional and caring manner.
  • Making and receiving calls to schedule appointments and follow up with members.
  • Representing the Health Plan at community events, outreach activities, and provider meetings.
  • Keeping accurate records of member interactions and program participation.

Minimum Qualifications

License Required: Possession of a Valid California Motor Vehicle Operator's License

Education: Possession of a high school diploma, G.E.D. equivalency or California High School Proficiency Certificate.

Experience: One (1) year of full-time, or its equivalent, experience performing marketing, sales, promotions, customer service or field service activities in a managed care program working with low income populations.

Selection Process

  • Application Filing and Evaluation: Applicants will be required to complete a supplemental questionnaire at the time of application. Applications will be evaluated to determine which candidates will move forward in the next phase of the recruitment process.
  • Training & Experience Evaluation: Candidates who clearly demonstrate that they possess the minimum qualifications will have their training and experience evaluated. The responses to the supplemental questions, at the time of filing, will be evaluated to determine each candidate's relevant education, training, and/or experience as presented on the application and supplemental questionnaire. (Weighted 100%)

The Human Resources Department may change the examination steps noted above in accordance with the Personnel Management Regulations and accepted selection practices.

CONVICTION HISTORY

DISASTER SERVICE WORKER

All Contra Costa County employees are designated Disaster Service Workers through state and local law. Employment with the County requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker-related training as assigned, and to return to work as ordered in the event of an emergency.
