Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team at Sturgeon Bay Health Services in Sturgeon Bay, Wisconsin.
This Full-Time or Part-Time Physical Therapist opportunity offers the chance to provide exceptional physical therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.
As a Physical Therapist (PT), you will work with adult and geriatric patients to improve mobility, strength, balance, endurance, and functional independence while collaborating with an interdisciplinary rehabilitation team focused on quality patient outcomes.
Responsibilities- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence.
- Perform therapeutic exercises, gait training, mobility training, and functional activities.
- Monitor patient progress and adjust treatment plans based on patient needs.
- Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
- Complete accurate and timely documentation according to company, state, and federal guidelines.
- Educate patients and families on mobility strategies, exercise programs, and safety techniques.
- Deliver compassionate, evidence-based physical therapy services.
- Graduate of an accredited Physical Therapy program.
- Current Wisconsin Physical Therapist (PT) license required.
- Skilled nursing or rehabilitation experience preferred but not required.
- New graduates are encouraged to apply.
