Shift :2nd FT 40 hrs. Tuesday - Saturday 4pm - 12am
Scope of Service: Provides comprehensive planning and active supports for the care and treatment of agency clients. Networks with area community providers and provides case management supports for the benefit of the clients. Supports management in the training and supervision of direct care staff, interns, and volunteers. Performs other duties as assigned.

Clinical Services and Supervision:

ESSENTIAL JOB FUNCTIONS:Program Management:
- Supports management in the administration of program standards and
related to the operation and provision of services.
- Supports management in monitoring costs and allocation of resources for program, in accordance with agency and client needs.
- Supports the management in implementing changes in the program reflecting identified agency priorities and strategic plan.
- Provides case management training and assists in assigning of caseloads.
- Assists in monitoring use and maintenance of the agency vehicle(s) assigned to program.
- Conducts supervisory spot checks in conjunction with management.
- Maintains property to ensure building/office is welcoming upon entrance.
- Works with staff to promote a clean, safe, and organized work environment for both staff and clients.
- Completes monthly data submission in accordance with the Department of
- Meets regularly with identified clients, inhome, as appropriate, to assess current status and progress.
- Maintains working relationships with identified community providers.
- Attends agency sponsored inservices, outside trainings, conferences and
Clinical Services and Supervision:
- Establishes rapport and working relationship with assigned clients.
- Develops and writes comprehensive community service plans with client,
- Provides oncall coverage in order to be available to staff in the program.
- Provides crisis intervention and support as clinically indicated.
- Attends and participates in monthly staff meetings, clinical rounds, and alltreater meetings.
- Participates in and conducts supervision as needed and identified by management.
- Facilitates/cofacilitates weekly psychoeducational groups.
- Networks with area community providers for the benefit of the clients.
- Adheres to policies and practices as outlined in the staff handbook and program operations manual.
- Consistently works in a respectful, constructive, and cooperative manner with fellow staff and providers.
- Upholds all Health Insurance Portability and Accountability Act (HIPAA)
- Models culturally competent behavior interacting with clients, professionals/providers, and fellow staff.
- Maintains client records as assigned and in accordance with contractual, licensing, and accreditation requirements.
- Supports management in the training and supervision of direct care staff for the clients on their caseload.
- Adheres to the standards of the Council on the Accreditation of Rehabilitation
- Participates in agency committee work when available or as permitted.
- Addresses concerns with staff or other professionals constructively and in a timely fashion.
- Schedules, attends and participates in agency meetings, team meetings, and interdisciplinary team meetings.
- Completes all training requirements as assigned by Agency.
- High School or equivalent considered with a minimum of three (3) years'
- or leadership experience preferred.
- Prior experience working with people with severe and persistent mental illness,
- Valid Connecticut Driver’s license is required. Scattered site and non24 hr.
- Current certification in First Aid/CPR certification or willingness to obtain
- Basic computer skills required.
- Knowledge of the basic principles of psychiatric rehabilitation is preferred.
- Knowledge of the community support system and resources is preferred.
