What are the responsibilities and job description for the Residential Aide - Health and Wellness position at Continuum of Care?
1st Shift: 25 Hours Monday - Friday 11AM - 4PM
Scope of Service: Assists individuals with daily personal routines and includes providing prompts for daily living skills, skill-building activities, and participation in groups. Provides companionship to residents and establishes a trusting and professional relationship. Works with licensed or visiting nursing personnel to monitor medical concerns and observe interaction with medications, as indicated. Reports any concerns or medical issues. Ensures living spaces are safe, sanitary, and well-organized. Leads group activities and outings. Collaborates with nursing, management, and coworkers to ensure residents receive the best quality services. Performs other duties as assigned.

- Health & Wellness activities such as yoga, meditation, nutrition, and some experience with arts and crafts.
- Establishes rapport and working relationship with clients.
- As a mentor, provides an invaluable role in the life of people who are newly attempting to create a life in the community by teaching, role modeling, and/or prompting to enhance strengths and build upon skill deficits in a significant way.
- Provides facetoface, handson supportive assistance, helping clients to maintain community living.
- Promotes clients’ strengths and abilities to improve socialization, recovery, selfadvocacy, development of natural supports, and maintenance of community living skills.
- Provides supportive companionship and discussions oriented in problemsolving.
- Engages clients in routine medical and community appointments as part of their recovery.
- Assists clients to access resources, travel planning and transportation efforts.
- Supports clients to achieve aspirations and goals and participates in recovery planning as indicated.
- Attends and participates in identified collaborative team and external community meetings as indicated.
- Maintains client records and reporting documentation as indicated by the agency.
- Attends and participates in monthly staff and supervision meetings.
- Facilitates/cofacilitates psychoeducational groups as indicated.
- Conducts regular apartment/house inspections and supports clients in maintaining overall cleanliness.
- Maintains a clean, wellorganized work environment and reports environmental concerns as identified.
- Participates in upkeep and sanitation of common areas, kitchens, bathrooms and so forth as identified.
- Participates in Food Bank activities as assigned and applicable.
- Participates in property upkeep and general office maintenance that supports workplace safety.
- Completes other designated environmental tasks as assigned.
- Adheres to policies and practices as outlined in staff handbook and program operations manual.
- Consistently works in a respectful, constructive, and cooperative manner with fellow staff and providers.
- Upholds all Health Insurance Portability and Accountability Act (HIPAA) regulations and secures all necessary releases of information for outside communication.
- Models culturally competent behavior interacting with clients, professionals/providers, and fellow staff.
- Maintains accurate electronic records and case files as assigned and in accordance with programming, contractual, licensing, and accreditation requirements.
- Adheres to the standards of the Council on the Accreditation of Rehabilitation Facilities.
- Participates in agency committee work when available or as permitted.
- Completes all trainings as required per program contract and agency policy.
- Attends agencysponsored inservices, outside trainings, conferences, and meetings to facilitate continued professional growth and ensure the quality of case management practices.
