Continuity of Care Providers, LLC – Fairfield County, Ohio.
Position Title: Direct Support Professional (DSP).
Status: Full-Time / Part-Time.
Reports To: Director of Operations.
Position Summary:
Direct Support Professionals (DSPs) provide essential, person-centered support to individuals with developmental disabilities. DSPs assist with daily living needs, promote independence, ensure health and safety, and support individuals in achieving their personal goals at home and in the community. All supports must uphold dignity, respect, empathy, and individual rights.
Essential Duties & Responsibilities: DSP responsibilities include, but are not limited to:
Direct Care & Daily Living Support:
- Assist individuals with Activities of Daily Living (ADLs), including bathing, grooming, hygiene, dressing, toileting, and mobility.
- Provide support with meal planning, meal preparation, feeding assistance, and general household tasks (cleaning, laundry, organization).
- Support individuals with maintaining a clean and safe living environment.
Health & Safety:
- Follow all health and welfare guidelines, infection control, and universal precaution procedures.
- Monitor individuals for changes in physical, emotional, or behavioral health and report concerns promptly.
- Implement safety plans, service plans, and risk mitigation strategies.
Community Integration & Social Support:
- Support individuals in participating in community activities, outings, events, and appointments.
- Encourage independence, skill-building, and choice-making.
- Foster positive relationships and meaningful daily engagement.
Documentation & Compliance:
- Complete all required documentation accurately and within required timeframes (EVV/Sandata, progress notes, incident reporting, etc.).
- Follow all policies and procedures, including HIPAA, Individual Rights, MUI/UI reporting, and person-centered planning requirements.
- Attend all required training and ongoing professional development.
Transportation:
- Provide transportation using your personal vehicle or agency vehicle (when applicable).
- Maintain valid driver's license, insurance, and safe driving record.
Qualifications:
- Must be at least 18 years old.
- High school diploma or GED.
- Valid driver's license, reliable transportation, and auto insurance required.
- Ability to pass a criminal background check, Rapback, and exclusion screenings.
- Must complete required new-hire DSP training per OAC
- Ability to lift, bend, and assist individuals as needed.

Schedule & Availability:
- 2 Full-Time & 2 Part-Time Positions Available.
- Weekday shifts: 1:30 PM – 6:30 AM.
- Weekend care: 24/7 rotating shifts.
- Hiring specifically for Fairfield County, Ohio.
