What are the responsibilities and job description for the Case Manager - RN position at ContinueCare Hospital at Baptist HealthP?
Directs and coordinates the integration of Case Management functions into the patient care, discharge, and home planning process with other hospital departments, external service organizations, agencies, and health care facilities. Coordinates the provision of clinical care to patients, families and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from health care services. At ContinueCARE Hospital, we take pride in the care we provide for patients with an average length of stay of 25 days in a variety of specialty programs. Our staff enjoy our small-hospital environment inside of a large acute care hospital.
Qualifications:
Graduate of an accredited nursing program. BSN preferred. Unrestricted RN license Case Management experience in an acute care environment preferred but not required.
