About Us
Contemporary Family Medicine has served the community for over 40 years, providing primary care for all types of patients. We offer acute care, chronic disease management, preventive care, and annual wellness exams. We specialize in managing conditions such as diabetes, hypertension, high cholesterol, and thyroid disease. Patients can also get lab work, x-rays, and ultrasounds in-house. We also provide well-woman care, men's health, hormone therapy, weight loss support, IV therapy.
About the Role
We're looking for a compassionate Nurse Practitioner or Physician Assistant to join our primary care team. You'll assess, diagnose and manage both acute and chronic conditions for about 20-25 patients per day. The role centers on preventive care, treatment planning and medication management. The ideal candidate would be an experienced provider (although new graduates with a strong background as an RN or EMT would be considered). Competitive compensation (bonuses), hourly salary (Depending Upon Experience) and benefits.
What You'll Do
- Conduct patient assessments and physical examinations
- Diagnose illnesses and develop individualized treatment plans
- Manage medications, including adjustments for patients with multiple prescriptions
- Manage chronic conditions such as diabetes, hypertension, high cholesterol and thyroid disease
- Provide preventive care, annual wellness exams and health education
- Counsel patients on lifestyle and health decisions.
- Coordinate care with physicians, nursing staff and specialists
- Keep accurate, timely documentation in the EMR
Qualifications
- Active Texas license as a Nurse Practitioner (APRN) or Physician Assistant
- National board certification (AANP/ANCC for NPs, NCCPA for PAs)
- Master's degree from an accredited NP or PA program
- At least 1 year of primary care experience preferred
- Strong clinical assessment and pharmacology knowledge
- Experience with chronic disease management and geriatric patients
- Proficiency with EHR systems
- Excellent communication and counseling skills, with empathy and sound judgment
