This position involves providing direct and indirect quality care to patients and families across all ages, under the supervision of the Hospice Clinical Supervisor and physician orders. The nurse manages a caseload within the hospice service area, coordinates care with the healthcare team, and promotes patient comfort and peaceful end-of-life experiences.

Responsibilities
- Assess and meet patients' individualized needs following the nursing process and Nurse Practice Act.
- Coordinate patient care activities with team members to ensure holistic, patient-centered support.
- Travel to patient locations within designated counties to deliver care.
- Oregon Registered Nurse License and current BLS certification.
- Minimum of 1 year medical/surgical, acute care, or long-term care experience.
- Experience in end-of-life (EOL) care preferred; recent oncology or hospice experience is a plus.
- Strong clinical assessment and coordination skills.
- Ability to work collaboratively within a healthcare team.
- Commitment to patient-centered, compassionate care.
