Role Overview: The Nurse Practitioner provides comprehensive, patient-centered care for individuals with serious illnesses, including hospice and palliative care. They perform various visit types such as Face-to-Face encounters, initial assessments, symptom management, and goals of care discussions, collaborating closely with physicians and interdisciplinary teams to ensure high-quality, compliant, and compassionate care.
Key Responsibilities: Conduct detailed patient assessments, manage diagnosis and treatment, prescribe medications (including controlled substances), order diagnostics, facilitate care transitions, and support advance care planning. Coordinate across settings, document accurately, and ensure compliance with regulatory standards. Communicate effectively with patients, families, and team members, supporting emotional needs and end-of-life decisions.
Qualifications & Skills: Must hold an MSN or higher, with an active Nurse Practitioner license, national certification, DEA registration, and prescriptive authority. Preferred experience in hospice, palliative care, or related fields, along with strong clinical assessment skills, knowledge of billing/coding, and proficiency with electronic records. Demonstrates compassion, flexibility, cultural sensitivity, and excellent communication abilities.
