Role: Diabetes Nurse Educator responsible for delivering diabetes self-management education following ADA and ADCES standards.

- Assess patient learning needs, goals, and barriers; tailor educational interventions accordingly.
- Lead individual and group sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring.
- Provide formal diabetes management instruction for inpatient and outpatient care.
- If qualified, deliver medical nutrition therapy services.
- Support ongoing diabetes management through follow-up and collaboration with healthcare teams.
- Stay current on diabetes technology, protocols, and industry developments.
- Document services in compliance with CMS and payer guidelines.
- Serve as a health resource for the community and participate in educational program development.
- Maintain necessary certifications, complete continuing education, and adhere to licensure requirements.
- Current RN or Registered Dietitian license; CDCES certification preferred or to be obtained within 2-3 years.
- Degree in Nursing, Dietetics, or related field; BSN preferred for nursing, Master's in Dietetics if applicable.
- Minimum 2 years of patient-focused educational experience preferred.
- Knowledge of diabetes management, patient education, and healthcare protocols.
