The Dietitian / Nutritionist will be an active participant in the interdisciplinary team and responsible to complete nutritional assessments, compile data, develop, and monitor a nutritional care plan. The Dietitian / Nutritionist will develop food serv

:

1. Participate as a nutritional consultant, with the interdisciplinary team in the development, implementation, and evaluation of the individual’s program plan and functions.

2. Asses dietary needs and prepares menus and develops specialized diets with engagement of individuals and staff interviews and with observations.

3. Develop an annual diet order and nutritional plan for each individual. Ensure the individuals diet order is co - signed by the individuals’ primary care provider. Communicate to the interdisciplinary team the diet order and update the team as needed with changes.

4. Provides diet and nutrition education, food safety and sanitation training to staff and also to individuals as appropriate

5. Maintains required documentation for compliance with regulations and monitors individual’s weights – reports any gain or loss as per set parameters to the nurse; along with changes in dietary habits or behavior.

6. Completes annual and/ or quarterly nutritional assessments as required by the individual, and develops a plan of care. Submits completed assessments / reports in advance of the scheduled interdisciplinary evaluation meetings.

7. Visits the various residential houses to monitor effectiveness of residents’ menus and diets and their meal preparation and consumption at least once a quarter.

8. Maintains effective written and verbal communications with agency professionals.

9. Interprets and utilizes pertinent current evidenced based practice related to nutritional care.

10. All other duties as assigned.

C. REQUIREMENTS:

1. Minimum of a Bachelor’s degree in Nutrition.

2. At least one (1) year of experience preferred.

3. Maintains current status as a Registered Dietician / Nutritionist with the Commission of Dietetic Registration including required CEU’s

4. Maintains current Basic Life support certification from American Heart Association

5. Competence in computer skills - Microsoft office, Electronic medical record experience preferred

6. Ability to travel to and access multiple locations throughout agency locations

7. Flexible work schedule with hours and/or days - works within approved hours. Schedule is provided and approved by the Director of Nursing at a minimum 2 weeks in advance

8. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims act, whistleblower protection and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
