What are the responsibilities and job description for the Registered Nurse - PRN (Norwalk, CT) position at Constellation Health Services?
Constellation Home Care is looking for a compassionate and skilled PRN/Per Diem Registered Nurse (RN) to join our team! This is a field-based RN position requiring someone to drive and cover an assigned territory treating patients in their homes, assisted living facilities, and in skilled nursing facilities.
The territory will cover Norwalk and Stamford, CT

Responsibilities:
- Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.
- Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.
- Develop the Plan of Care in coordination with all health care professionals assigned to the case.
- Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Report to the Clinical Manager on a daily basis.
- Provide documentation using the provided iPad within the required time frame.

Qualifications:
- Registered Nurse Licensed in the state of employment, graduate of an accredited RN program.
- At least 1-2 years of RN experience is a MUST; Home care preferred
- Oasis experience is strongly desired
- Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
- Have reliable transportation and be willing to commute to provide the best in-home patient care.
- Some weekend days are required, and summer and winter holiday commitments as needed.
