Compensation and Benefits:
- Earning potential: $115,000–$200,000 (salary based on experience easily achieved monthly bonuses)

Key Responsibilities
- Deliver person-centered care using evidence-based practices
- Collaborate with interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow-up care
- Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Compliance with facility, state and federal regulations/protocols.
Qualifications:
- Active license as a Nurse Practitioner (NP)
- Minimum 2 years of post-certification experience required
- ACHPN Certified Preferred (Advanced Certified Hospice & Palliative Nurse)
- Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
