What are the responsibilities and job description for the HHA Spanish Speaking position at Constant Care Health Services LLC?
Constant Care Health Services is a Licensed Home Care Agency that provides Excellent Services to our Community. We are currently seeking FT and PT HealthCare Heros to join our Constantly Caring team.
QUALIFICATIONS:
Current Home Health Aide Certificate in the State of New York
RESPONSIBILITIES:
- Assist with patients Personal Care needs: personal hygiene, bathing, dressing/undressing, and ambulation together with any household task (eg making beds, changing linen, laundry, light housekeeping, etc) essential to the patients’ health in accordance with the patient's individualized Plan of Care.
- Plan/Prepared nutritious meals and assist in feeding patients as directed. Document services provided in accurate and timely manner in accordance with company policy and personal care plan.
- Maintain Confidentiality at all times
- Report any changes in the patient’s condition or incidents to RN Supervisor
- Carry out other duties
- Accompany patients to their doctor visits
