Conscience Community Network (CCN) is a collaboration of Illinois not-for-profit service providers. CCN’s mission is to improve the lives of at-risk youth and families involved in the child welfare and criminal/legal systems. CCN and its network partners implement Wraparound programming in collaboration with youth and families to address unmet needs. This role collaborates with both of CCN’s programs. One program is the High Fidelity Wraparound program which facilitates the Wraparound process with families that are involved with the child welfare system and the children and youth have complex emotional, behavioral, or mental health needs. The other program is the Dually Involved Youth program which facilitates the Wraparound process with youth that are involved in both the child welfare and criminal/legal systems.

Job Summary
The Program Specialist provides ongoing programmatic support to CCN’s network of service providers and is part of the administrative team of Conscience Community Network (CCN). This role includes documentation review and processing, feedback to providers regarding their implementation of the Wraparound model, and utilizing Wraparound expertise to make programmatic improvements.
Essential Duties and Responsibilities
- Completes observations of Wraparound meetings and uses observation tools to document Wraparound fidelity.
- Reviews and processes documentation such as Flex Fund requests and Status Change requests.
- Tracks youth time in the programs to ensure youth are enrolled for an appropriate length and facilitates the extension process, if necessary.
- Assists with specified steps of the intake process for both programs.
- Shares information about the programs with external entities to recruit referrals and educate the field.
- Utilizes Wraparound expertise to strategize ways to improve program delivery to youth and families within CCN programs.
- Identifies gaps in service array and coordinates with partners or external providers to fill gaps.
- Other duties as assigned.
Education: Bachelor’s Degree from an accredited university in psychology, social work, counseling, education or similar-related field.
Experience: 3 years of experience, ideally with care coordination, supervision/consultation, and/or facilitating family team meetings.

- Eagerness to make ongoing improvements in personal tasks and program wide systems, to ensure the most efficient and effective workflow is designed and followed.
- Effective verbal and written communication skills with ability to convey information to a wide variety of audiences in a clear, concise manner. Strong knowledge of spoken and written English sufficient to obtain, provide, or exchange information. Knowledge of spoken and written Spanish is not required but is a value add.
- Knowledge of Wraparound is not required but highly valued.
- Ability to exercise discretion and ensure confidentiality.
Local and regional travel, with occasional overnight stays, is required for this position.
