ConnextCare is seeking a full-time Registered Nurse (RN) Care Manager to join our dedicated team on-site in Fulton, NY. The RN Care Manager will coordinate and manage patient care, collaborating closely with a multidisciplinary team to ensure the delivery of high-quality medical services. Responsibilities include performing patient assessments, creating and managing care plans, facilitating communication between patients and other healthcare providers, and providing direct nursing care. The role also requires ongoing evaluation of patient progress and the implementation of strategies that prioritize patient well-being.
- Strong foundation in Nursing, Medicine, and Patient Care
- Experience in Nursing Management and coordinating care for diverse patient populations
- Skills in Critical Care Medicine and managing complex patient cases
- Exceptional organizational, communication, and problem-solving abilities
- Valid RN license in New York State required
- Experience with electronic health record systems is a plus
- Understanding of culturally competent care with a patient-centered approach
- Bachelor's degree in Nursing or equivalent educational background preferred
