Board-Certified Family Nurse Practitioner

Job Summary

We are seeking a dedicated Nurse Practitioner to join our family medicine team, providing comprehensive, patient-focused care to individuals and families across all stages of life. In this role, you will serve as a primary healthcare provider, performing detailed patient evaluations, diagnosing and treating a broad spectrum of acute and chronic conditions, and delivering preventive health services. You will be instrumental in developing personalized care plans that address both immediate health concerns and long-term wellness goals.

Duties

• Conduct thorough patient assessments, including medical histories and physical examinations.
• Diagnose and develop treatment plans for various acute and chronic health conditions.
• Prescribe medications and other therapies in accordance with state regulations.
• Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Utilize EMR/EHR systems to document patient information accurately and efficiently.
• Educate patients on health management, medication adherence, and preventive care strategies.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal health outcomes.
• Maintain compliance with healthcare policies, safety standards, and confidentiality regulations.

Experience

• Proven experience as a Nurse Practitioner or in a similar advanced practice nursing role.
• Proficiency in using EMR/EHR systems for documentation and patient management.
• Strong collaboration skills to work effectively within multidisciplinary teams.
• Excellent communication skills for patient education and team coordination.
• Valid licensure and certification as a Nurse Practitioner

Pay: $50.00 - $55.00 per hour

Expected hours: 40.0 per week
