We are currently seeking a Health Homes Care Manager to support individuals with complex behavioral and medical needs by coordinating care, connecting clients to community resources, and promoting overall wellness.
Position Overview
The Health Homes Care Manager is responsible for coordinating medical, behavioral health, and social services for clients enrolled in the Maryland Behavioral Health Health Homes Program.
Care managers work closely with clients, providers, and community partners to improve health outcomes and ensure individuals receive comprehensive, coordinated care.
KEY RESPONSIBILITIES:
- Conduct Initial Health Homes Assessments and develop individualized care plans.
- Complete monthly care management activities, including at least two client contacts per month.
- Coordinate care between medical providers, behavioral health providers, and community resources.
- Monitor client progress toward health and wellness goals.
- Assist clients with scheduling medical appointments and accessing healthcare services.
- Communicate with primary care providers and other healthcare professionals as needed.
- Document care management activities in accordance with Maryland Health Homes program requirements.
- Support clients with addressing social determinants of health such as housing, food access, and transportation.
- Collaborate with CTM’s PRP staff and OMHC clinicians to support integrated care.
- Maintain accurate documentation in the agency’s electronic health record.
QUALIFICATIONS:
- Possess a Master’s degree from an accredited university in a related field
- Be licensed as a Registered Nurse with the Maryland Board of Nursing
- Experience using computer software.
- Experience in the mental health or human services field.
- Experience working with individuals with serious mental illness or chronic medical conditions (Preferred but not required).
- Able to manage a flexible but committed schedule.
- Able to pass a criminal background check.
REQUIRED LICENSE/CERTIFICATION:
- Professional License
- Driver’s License
- Proof of current Auto Registration, Inspection and Insurance

COMPENSATION
This is a contractual position with flexible scheduling. Compensation may include:
- Monthly care management rate per enrolled client.
- Additional compensation for completing Health Homes assessments.
