Connections Care Services is a supported living services provider that serves adults with developmental disabilities in a home setting. We are looking for highly dependable, motivated, compassionate individuals for the position of Direct Support Staff/Caregiver in the Tulare/Visalia area. Direct Support Staff play a vital role to assists clients with intellectual, cognitive, or developmental disabilities to achieve their highest level of independence. In this vital role, you will assist clients with daily activities, ensure their safety, and promote independence within a nurturing environment.
Requirements: 18 years of age or older, high school diploma/GED, valid driver's license, acceptable DMV driving record, ability to communicate effectively (verbally and written). CPR/Fist Aid certification preferred but will be provided. Position require flexibility in assignment of work locations.
Duties
Assist clients with personal care tasks including bathing, dressing, grooming, personal hygiene and grooming, feeding, ambulation, toileting, medical monitoring, health care related tasks, and mobility assistance
Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately
Performing home management functions such as light housekeeping, laundry, bed making, and cleaning
Ensuring client safety and maintaining a safe environment including deescalation of behaviors
Encouraging self-help activities
Planning meals, shopping for groceries, preparing and serving food/meals, feeding and clean-up
Transporting clients, running errands, accompanying client to appointments
Support medication administration and assistance following HIPAA guidelines and care plans
Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients
Document client observations, care activities, and any incidents clearly and thoroughly in compliance with privacy regulations
Provide companionship and emotional support to clients, fostering a warm atmosphere
Ensuring client safety and maintaining a safe environment including deescalation of behaviors
Collaborate with healthcare professionals to implement personalized care plans for residents with dementia, autism, or other disabilities
Skills
Proven experience in home health, assisted living, nursing homes, or group home settings
Knowledge of patient care techniques including CPR, first aid, and medication administration
Familiarity with working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's or memory care needs
Ability to perform heavy lifting safely using equipment like Hoyer lifts or resident lifts
Strong observational skills for patient monitoring and recognizing changes in condition or behavior
Excellent communication skills to interact effectively with clients, families, and healthcare teams
Ability to follow detailed care plans while adhering to HIPAA regulations for privacy and confidentiality
Experience in meal preparation, cleaning, and maintaining a safe environment for residents
Compassionate attitude combined with patience when working with seniors or individuals with complex needs such as autism or hospice care
