Social Worker (Part Time)

We are a care home placement agency working with various Managed Care Plans as a California Advancing and Innovating Medi-Cal (CalAIM) Community Support (CS) Provider for Skilled Nursing Facility (SNF) Diversion and Transition to Assisted Living. In California, assisted living homes, also known as residential care facilities for the elderly or RCFEs, are either small 4-6 bed residential board and care homes or large 100 bed assisted living homes. The CS is a Medi-Cal waiver program similar to the Assisted Living Waiver (ALW).

1) The CalAIM CS social worker position involves monthly in-person quality check visits to CS members placed at CS RCFEs and to input the completed quality check assessment tool and any incident reports into our online database. The pay is $45 for each client monthly visit.

2) The position will also entail occasional in-person visits to members (only for Kaiser) for Individual Service Plan (ISP) assessments to determine the tiered level of care. This is a two hour process paid at $45/hour ($90 for the two hours).

We will also reimburse $20 for daily gas expenses

The job is very flexible, client RCFE visits can be done on weekends and the marketing during the weekday is on your own schedule.

Job Requirements:

The social worker must have a Master's degree in social work plus two years of related work experience.

The job applicant must:

Be friendly and outgoing

Have own car and transportation.

Have own computer or laptop with internet connection.

Be familiar with computers and data entry
