Case Manager
Full Time Position
Hours: Minimum of 40 hrs. per week
Pay Rate: $23 or DOE

- Coordinate & Manage schedules
- Ensure full compliance with State regulations and can follow Executive Directors guidelines
- Prepare, coordinate, and review individualized plans of care to support clients
- Maintain accurate, organized and current records/documentation
- Oversee and manage house accounts

Pay: From $23.00 per hour
Expected hours: No less than 40 per week
