Our services include Supported Independent Living, personal care, mental health support, nursing care, recovery coaching, community access, respite care, support coordination, and medication assistance.
This is a full-time on-site role for a Disability Support Worker located in Epping, NH. The Disability Support Worker will provide daily living assistance, personal care, mental health and psychosocial disability support, and community access services. Key responsibilities include assisting with hygiene, meals, household tasks, clinical care for high needs, goal-focused coaching, and organizing social outings or transport. The role requires close collaboration with healthcare professionals to ensure high-quality care tailored to individual needs.
- Experience in providing personal care and daily living assistance
- Skills in mental health support and behavioral management
