Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs, recognizing changes, and reporting findings accurately using electronic medical record (EMR) systems
- Support with medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards
- Provide specialized care such as Alzheimer's and dementia care, memory care, end-of-life support, and hospice assistance
- Operate patient transfer methods safely using equipment like Hoyer lifts or other transfer devices; assist with patient transfers and mobility exercises
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients
- Document medical records accurately and maintain detailed medical documentation for each client’s care plan
Qualifications
- Prior experience working with individuals with developmental disabilities, geriatrics, or in assisted living settings is highly preferred
- Knowledge of medical terminology, catheter care, CPR, first aid, and basic life support (BLS) certifications required or obtainable upon hire
- Experience with EMR systems and medical documentation practices essential for accurate record keeping
- Ability to lift heavy objects safely and perform physically demanding tasks as needed for patient transfer and mobility assistance
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Familiarity with home health care regulations, HIPAA compliance, and patient confidentiality standards necessary for quality care delivery
- Previous experience in home health agency settings or long-term care environments is advantageous
