Healthcare Marketing Liaison
East Bay Area (Alameda County & Contra Costa County, CA) – Field-Based Role
About:
We are a licensed home health and private duty agency serving Northern California. We provide comprehensive in-home care and post-acute support services, working closely with healthcare providers to ensure continuity of care.
We are currently building a lean, growth-focused referral model centred on strong healthcare community relationships.
Position Overview:
We are seeking an experienced Healthcare Marketing Liaison / Community Referral Partner to expand our referral network across the East Bay Area, specifically Alameda County and Contra Costa County.
This is a field-based, in-person business development role focused on relationship building—not digital marketing or social media.
Key Responsibilities:
- Develop and maintain referral relationships across the East Bay healthcare community
- Conduct in-person visits to healthcare facilities and providers, including:
- Hospitals and discharge planners
- Skilled nursing facilities
- Rehabilitation centres
- Physician offices
- Case managers and care coordinators
- Workers’ Compensation representatives
- Present and promote agency services to referral sources
- Generate new patient referrals
- Maintain an active referral pipeline
- Track and report weekly outreach activity
- Complete consistent documented referral visits per month
Ideal Candidate | We are looking for someone who has:
- Experience in Home Health Marketing, Hospice Liaison, or Healthcare Sales
- Existing relationships within Alameda and/or Contra Costa County healthcare networks
- Strong understanding of discharge planning and referral workflows
- Self-driven, accountable, and results-oriented mindset
- Ability to manage independent, field-based work
Location Requirement:
Must be based in or near the East Bay Area (Alameda County & Contra Costa County, CA)
Pay: $60,000.00 - $110,000.00 per year
