Job Requirements- Education Requirements: High school diploma or equivalent; additional healthcare-related education preferred
- Certifications Requirements:
- Basic Life Support (BLS)
- Driver's License and auto insurance
- Reliable vehicle for travel between patient locations
- Experience Requirements:
- Minimum of 2 years of hospice experience preferred; willing to consider candidates with less experience
- Experience in patient assessments and end-of-life symptom management
- Familiarity with EMR systems, specifically EPIC
- Other Requirements:
- Excellent communication and patient education skills
- Comfortable working independently in private homes, SNFs, and assisted living facilities
- Ability to explain hospice care and expectations clearly to patients and families

- Provide skilled hospice care to adult patients in private homes, SNFs, and assisted living facilities, ensuring comfort and dignity.
- Complete two patient admissions during each shift, including assessments, education, and care planning.
- Collaborate effectively with physicians, liaisons, and case managers to coordinate patient care and adjust treatment plans as needed.
- Manage symptoms related to end-of-life conditions, including pain and breathlessness, with appropriate medications and interventions.
- Document all patient interactions, assessments, and interventions accurately within the EPIC EMR system, ensuring compliance and continuity of care.
