• Education Requirements: High school diploma or equivalent; additional healthcare-related education preferred
• Certifications Requirements:
  • Basic Life Support (BLS)
  • Driver's License and auto insurance
  • Reliable vehicle for travel between patient locations
• Experience Requirements:
  • Minimum of 2 years of hospice experience preferred; willing to consider candidates with less experience
  • Experience in patient assessments and end-of-life symptom management
  • Familiarity with EMR systems, specifically EPIC
• Other Requirements:
  • Excellent communication and patient education skills
  • Comfortable working independently in private homes, SNFs, and assisted living facilities
  • Ability to explain hospice care and expectations clearly to patients and families

• Complete two patient admissions during each shift, including assessments, education, and care planning.
• Collaborate effectively with physicians, liaisons, and case managers to coordinate patient care and adjust treatment plans as needed.
• Manage symptoms related to end-of-life conditions, including pain and breathlessness, with appropriate medications and interventions.
• Document all patient interactions, assessments, and interventions accurately within the EPIC EMR system, ensuring compliance and continuity of care.
