Position Summary

The Project Manager is responsible for the implementation, program management, and staff supervision of Community Health Workers (“CHWs”) to improve health outcomes in the North Philadelphia Health Enterprise Zone (“HEZ”). The program provides interventions to impact the social and environmental barriers to health for clients in multigenerational households, to include children and older adults, and at least one of these indicators: frequent utilizers of emergency health services, those with frequent hospital admissions, and/or not meeting managed care standards for their medical condition(s) (focus on Hypertension, Diabetes, HIV, and Asthma). 


The Project Manager oversees the day-to-day program activities, training for Community Health Worker’s, service documentation, contract goals, reports, and program quality assurance. This position coordinates and hosts internal monthly case consultations and manages a reduced client caseload. The Project Manager supervises two Community Health Workers and works with Congreso’s Healthcare Services points of contact to obtain client referrals. 


This position reports to the Vice President of Healthcare ServicesThis is a Hybrid position with onsite work conducted at Congreso's main office and/or a client's home.

Program Management 

  • Tracks, documents, and submits to the VP of Health Care Services for review, all updated programmatic monthly, quarterly, annual, and ad hoc reports. This includes monthly reporting that tracks the services provided within the contracts and provides guidance on goal attainment. 
  • Is responsible for caseload assignments and covers CHW caseloads, when needed, manages a reduced caseload and tracks funder requirements.
  • Coordinates and hosts internal monthly case consultations and as needed with points of contacts to provide client updates and next steps. 
  • Oversees the quality and quantity of services and program results to meet and/or exceed the funder requirements, Congreso's mission, and improves the client’s overall quality of life.
  • Manages the program using Congreso’s Efforts-to-Outcomes database to help assess whether workplan activities are meeting the contract goals.
  • Works with the VP of Health Care Services and the Learning and Evaluation Analyst in the development, coordination, execution, tracking, and reporting of program deliverables.
  • Coordinates content development of training and resources for the program. 

Quality Assurance

  • Develops and implements quality assurance plans for the program(s), which includes mechanisms and evaluation tools to assess quality of services and quality control.
  • Organizes and implements an active consumer advocacy group through the program’s active clients or otherwise.
  • Develops an on-going feedback process to be implemented into the quality assurance plans, and to include client chart audits.

Staff Management Duties

  • Is responsible for providing quality supervision to the Community Health Workers (CHW, including a minimum of bi-weekly supervisory meetings, annual performance reviews and work plans as deemed necessary.
  • Adheres to agency supervisory policies regarding personnel management, engages Human Resources as appropriate, and coaches staff development.
  • Regularly directs the work, schedules, and provides development opportunities and training to department staff.
  • Conducts staff performance evaluations of all assigned at the end of each probationary period, annually, and adheres to agency personnel policies.
  • Performs management functions and/or makes recommendations for the department such as hiring, termination, performance evaluations, disciplinary counseling, salary adjustments, workload distribution, communication, recognition activities, and employee relations, engaging Human Resources as appropriate.
  • Identifies and coordinates training that enhances staff skills and complies with funder continuing education requirements and works with the VP of Healthcare Services to implement training.

  • Ability to understand, acknowledge, and assess client needs to provide a timely, trauma informed approach in service.
  • Ensures the appropriate handling of sensitive information and understanding for issues such as confidentiality, domestic abuse, child abuse, and mandated reporter requirements.
  • Adheres to all Congreso, departmental, programmatic, and security policies, procedures, and guidelines.
  • Attends all Congreso trainings and in-services as required, including relevant professional groups to maintain professional, requisite knowledge and licensure.
  • Attends, participates, and facilitates relevant staff meetings to promote communication and execution of goals.
  • Ability to effectively use standard office equipment, including a variety of common software programs such as Office365, UNIDAD, and other customized databases.
  • Follows through on assignments, works in a timely manner, within established deadlines, and completes special projects specific to the function of the department, as needed or as directed by supervisor.
  • Demonstrates ability to work independently and collaboratively with diverse groups, in a compassionate, courteous, and respectful manner that fosters an inclusive environment.
  • Strong written and verbal communication skills including effective and respectful communication with individuals and groups.
  • Bilingual (Spanish/English) written and verbal skills preferred.

Supervisory Responsibilities:

This position directly supervises Community Health Workers.

 

Minimum Experience, Education, and Qualifications

Minimum Education: 

Bachelor’s degree in public health or public administration or related field, a plus. 5 or more years of related field experience in lieu of education considered.


Minimum Experience: 

Minimum of 3 years’ experience in public health, project coordination, or a related area, required.

A minimum of 3 year supervisory experience, a plus.

Experience with management of federal contracts and non-profit evaluation techniques and tools, a plus.

 

Certification/License: 

Child abuse and Criminal Clearances must be obtained prior to hire start date and updated every 2 years.

Valid driver’s license, a plus.

Salary : $43,366 - $60,000
