Summary :
The Wound Care Specialist for home care services is responsible for providing clinical oversight for all wound care provided within the home health and hospice department. The Wound Care Specialist will establish standards of care and clinical competencies to meet patient needs and wound care, incontinence, and ostomy training programs to ensure visiting staff have clinical competence and expertise. The Wound Care Specialist provides consultation for complex wounds, manages, develops, and monitors the agency wound care program to improve clinical outcomes.
Position Reports To : Home Health and Hospice Manager
Essential Functions :
Create agenda and lead the monthly / bimonthly wound care meetings.
Assess and evaluate new products pertaining to wound care services :
Prepare evaluations and present products to the Value Analysis Committee.
Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.
