Summary :

The Wound Care Specialist for home care services is responsible for providing clinical oversight for all wound care provided within the home health and hospice department. The Wound Care Specialist will establish standards of care and clinical competencies to meet patient needs and wound care, incontinence, and ostomy training programs to ensure visiting staff have clinical competence and expertise. The Wound Care Specialist provides consultation for complex wounds, manages, develops, and monitors the agency wound care program to improve clinical outcomes.

Position Reports To : Home Health and Hospice Manager

Essential Functions :

Wound assessment and management : conduct thorough consultation or assessment of patients with complex wounds, including pressure ulcers, surgical wounds, and chronic ulcers and assist in developing appropriate individualized care plans for treatment and prevention.

Incorporate evidence based clinical decision making to select and utilize appropriate patient interventions in accordance with plan of care and modify plan of care as appropriate based on patient progress / response to interventions.

Stays current with advancement in wound and ostomy care through research and applying evidenced based practices to enhance patient care and clinical outcomes.

Monitor and evaluate supply utilization and management, evaluate advanced wound care products.

Establish and maintain wound treatment algorithms that support current evidence based best practice.

Provide education to patients, caregivers and other staff members as appropriate.

Provide supervision as needed to nursing students.

Collaborate and communicate effectively with other team members including physicians, surgeons, and other specialists, case managers, nurses, and therapy staff, including participation in discharge planning in home health and hospice settings.

Screen for, and make referrals to, other disciplines when indicated.

Coordinate training in wound care services and interventions :

Create agenda and lead the monthly / bimonthly wound care meetings.

Strengthen wound care team relationships by educating, coaching and facilitating teamwork to improve and coordinate wound care services to the patient.

Assess and evaluate new products pertaining to wound care services :

Prepare evaluations and present products to the Value Analysis Committee.

Train clinical team on new products / services.

Participate in new hire department orientation and training processes including, but limited to, documentation, policies, procedures, guidelines, initial and ongoing competency assessment of skills.

Provides mentoring and clinical support to coworkers and support staff.

Promotes active participation in quality improvement projects.

Identifies process trends, variances, and problem areas and works with the department Manager and unit Educator. Helps implement process improvements.

Schedules and organizes continuing education and in-services to direct care staff.

Assists with writing new protocols, policies and guidelines for the home care services department as needed. Helps to ensure protocols, policies and guidelines are reviewed and current.

Required to maintain current BLS certification.

Other duties as assigned.

Qualifications : Required :

Bachelors Degree or higher in Nursing or obtain within 1 year of hire.

Current wound and / or ostomy certification or obtain within 1 year of hire.

3 years of clinical wound care experience.

Valid drivers license and auto insurance.

Access to a reliable vehicle.

Desired :

1 year of home health or hospice experience.

Current WOCN, wound and / or ostomy certification.
