Ascend Mental Health is seeking a passionate and engaged Rehabilitation Specialist who thrives on excitement and quality service delivery to clients. The Rehabilitation Specialist oversees the psychiatric rehabilitation program and ensures that the service provided by our Direct Care Counselors reflect the need of the clients served by the program. The Rehabilitation Specialist is responsible for providing oversight for the daily program rehabilitation services, and collaborates with our Program Directors.
Duties:
- Develop individual rehabilitation plans alongside Rehab Team
- Review and approve monthly progress reports for Psychiatric Rehabilitation Program (PRP) clients.
- Conduct an initial and periodic review of client’s existing Individual Rehabilitation Plan (IRP) with Program Director and assigned Direct Care Staff member. Develop new IRPs if needed
- Review and approve contact notes from Direct Care Staff members to monitor client progress, and identify clients that require referrals for additional treatment
- Assist Program Director in identifying staff training needs
- Regularly review and audit documentation for quality assurance
- Provide supervision and direction over the PRP program
Qualifications: Must be a licensed mental health professional with a minimum of 2 experience working with emotionally disturbed youth
