Duties
- Conduct comprehensive assessments of clients' medical, social, and emotional needs to develop personalized care plans.
- Coordinate discharge planning and facilitate smooth transitions from hospital to home or other care facilities.
- Collaborate with healthcare providers, social workers, insurance companies, and community resources to optimize client care and resource utilization.
- Perform utilization review and management to ensure appropriate level of care while maintaining compliance with policies .
- Utilize Electronic record keeping to document clients' interactions, progress notes, and care plans.
- Qualifications
- BS/BA in human services or related field or a RN or LPN.
- Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.
