Clinical Consulting Services is seeking a dynamic and relationship-driven Float Clinical Care Coordinator to support our skilled nursing facility partners throughout the Lancaster and Harrisburg, Pennsylvania region.
This position is ideal for an experienced healthcare professional who enjoys working in a fast-paced environment, building strong referral relationships, and helping patients access exceptional post-acute care. As a regional Float Clinical Care Coordinator, you'll serve as a trusted clinical resource across multiple facilities, supporting admissions, census growth, and seamless transitions of care.
ResponsibilitiesProvide clinical liaison support to multiple skilled nursing facilities throughout the Lancaster/Harrisburg region.
Build and maintain strong relationships with hospitals, physicians, case managers, discharge planners, social workers, and other referral sources.
Evaluate referrals and collaborate with facility leadership to coordinate timely admissions.
Educate referral partners on skilled nursing services, rehabilitation programs, and specialty clinical offerings.
Identify opportunities to increase census through strategic relationship development and community outreach.
Support facilities with admission coordination, clinical communication, and transition planning.
Collaborate with interdisciplinary teams to ensure outstanding patient experiences and continuity of care.
Monitor referral activity and maintain accurate documentation.
Represent Clinical Consulting Services professionally at hospital meetings, networking events, and community outreach opportunities.
Travel throughout the Lancaster/Harrisburg market to support assigned facilities.
Current Pennsylvania RN or LPN license required.
Minimum of two years of experience in skilled nursing, case management, admissions, clinical liaison, healthcare marketing, or a related field preferred.
Strong knowledge of Medicare, Managed Care, discharge planning, and skilled nursing admissions.
Outstanding communication, presentation, and relationship-building skills.
Self-motivated with excellent organizational and time-management abilities.
Valid driver's license, reliable transportation, and willingness to travel throughout the region.
Experience with EMR and CRM systems is a plus.
