Overview
We are seeking a compassionate and highly skilled Hospice Registered Nurse (RN) to provide exceptional end-of-life care to patients in a supportive and respectful environment. The ideal candidate will have extensive experience in various clinical settings, including critical care, palliative medicine, geriatrics, and home health. This role involves comprehensive patient assessment, symptom management, medication administration, and coordination with multidisciplinary teams to ensure quality comfort care. The Hospice RN will utilize advanced clinical skills and EMR/EHR systems to deliver personalized care aligned with patient needs and family preferences.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations.
- Develop and implement individualized care plans in collaboration with physicians, families, and interdisciplinary teams.
- Administer medications and injections, including IV infusions, sedation, and pain management therapies such as opioid titration and botulinum toxin treatments.
- Manage complex symptoms related to hospice care such as acute pain, dyspnea, nausea, and agitation using appropriate interventions.
- Provide education on disease processes, medication use, advanced directives, and end-of-life planning to patients and families.
- Coordinate patient care across various settings including home care, assisted living facilities, nursing homes, hospitals (Level I & II trauma centers), PICU, NICU, outpatient clinics, and inpatient units.
- Utilize EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for documentation of medical records and medical documentation standards compliant with HIPAA regulations.
- Perform laboratory specimen collection/processing when necessary and ensure infection control protocols are followed diligently.
- Engage in discharge planning and case management to facilitate smooth transitions between care settings.
- Participate in ongoing education related to hospice & palliative medicine, infectious disease care, trauma medicine, geriatrics, behavioral health, and other specialized fields as needed.
Skills
- Must have experience using WellSky (Consolo) charting system is preferred.
- Strong knowledge of physiology, anatomy, pharmacology related to pain management and symptom control.
- Skilled in IV insertion/infusion therapy; aseptic technique; airway management; ventilator care; catheterization; ostomy care; gastrostomy tube feeding; sterile processing; phlebotomy; and specimen collection/processing.
- Experience with complex patient populations including pediatrics (neonatal ventilator), geriatrics (memory/dementia/dementia care), behavioral health patients with developmental disabilities or mental health needs.
- Ability to perform physical examinations and diagnostic evaluations accurately while adhering to infection control standards.
- Excellent communication skills for patient education on discharge planning and family support.
- Knowledge of Workers' Compensation law applicable to occupational health cases is a plus.
- Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are advantageous.
- Familiarity with post-acute care environments including sub-acute care settings or outpatient clinics is desirable.
