Job Description

SUMMARY

The Nurse Practitioner (NP) is responsible for providing primary and preventative healthcare services to patients of all ages within the Three Rivers Health Clinic. The NP will work collaboratively with a multidisciplinary team to address the physical, emotional, and social health needs of the tribal community, while honoring and integrating traditional healing practices where appropriate.

Principal Activities & Responsibilities

  • Provides diagnosis, treatment, and management of acute and chronic illnesses.
  • Conducts physical exams, health screenings, and immunizations across the lifespan.
  • Prescribes medications and treatments in accordance with federal and tribal regulations.
  • Promotes disease prevention and healthy lifestyle practices through patient education and outreach.
  • Collaborates with tribal leaders and community health representatives to ensure culturally appropriate care.
  • Documents all patient encounters in the electronic health record (HER) accurately and timely.
  • Participates in quality improvement initiatives, case reviews, and community health planning.
  • Serves as a clinical resource for medical assistants, nurses, and other staff.
  • Maintains knowledge of best practices in tribal health and indigenous health disparities.
  • Upholds patient confidentiality and clinic policies in accordance with HIPAA and tribal law.
  • Demonstrates professionalism in all interactions and maintains positive, respectful relationships with patients and tribal members.
  • Other duties as assigned.

Level Of Authority & Restrictions

  • No supervisory authority.

WORKING CONDITIONS & ENVIRONMENT

  • Combination of exam rooms and medical offices.
  • Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinic environment.
  • Work involves considerable exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.
  • Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
  • Will work with blood or blood-borne pathogens and will require OSHA training.

PHYSICAL & MENTAL DEMANDS

  • Must be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.
  • Requires standing and walking for extensive periods of time.
  • Occasionally pull, lift and/or carry items weighing up to 50 pounds.
  • Requires corrected vision and hearing to normal range.
  • Requires working under stress in emergency situations and irregular hours.

Required Experience

MINIMUM JOB REQUIREMENTS

  • Must be at least 26 years of age.
  • Must have a valid Driver's license and be able to be insured to drive Tribal Government vehicles.
  • Masters or doctorate degree in Nursing from an accredited program.
  • Current, unrestricted Nurse Practitioner license in the state of Oregon.
  • Minimum three (3) years of nursing experience, including two (2) years as a nurse practitioner in an outpatient clinic.
  • National Board Certification as a Family Nurse Practitioner (FNP-C or FNP-BC) preferred.
  • DEA license and prescriptive authority (or eligibility to obtain).
  • BLS/CPR certification required; ACLS preferred.
  • Knowledge of patient care standards, clinical regulations, and safe work practices.
  • Proficient in using medical equipment and instruments for patient care.
  • Skilled in clinical assessment, including taking medical histories and interpreting findings.
  • Strong communication skills and ability to build trust with diverse populations.
  • Understanding, or willingness to learn about, tribal sovereignty, traditions, and healthcare practices.
  • Ability to communicate clearly and effectively in English, verbally, in writing or by other acceptable means.
  • This is a safety sensitive position and will be subject to pre-employment drug testing and criminal history background check, which will include fingerprinting.
  • Must have employment eligibility in the U.S.
  • Indian preference will be observed in the hiring process.
