We are seeking a highly motivated and detail-oriented Medical Assistant (MA) to manage our growing panel of CCM and RPM patients as a Patient Care Coordinator. This is a crucial, non-face-to-face role focused on proactive patient communication, monitoring remote data, and coordinating care under the general supervision of our physicians.
Key Responsibilities
- Facilitate communication between the patient and other healthcare providers, including specialists, home health, and community resources.
- Conduct regular, proactive outreach calls to monitor patient adherence to the care plan, assess barriers to compliance, and provide motivational coaching.
- Assist in the creation, maintenance, and distribution of the patient’s comprehensive, patient-centered care plan.
- Assist the provider with medication reconciliation and ensuring patient adherence to their treatment regimen.
- Instruct patients on the proper use and setup of remote monitoring devices
- Escalate critical physiological data and complex clinical concerns to the supervising provider immediately.
