Position Summary
The Wellness Coordinator is responsible to assure the provision of high-quality care, compliance, and customer service standards on his/her assigned unit(s) making sure that the medical, physical, social, and emotional needs and general welfare of residents are met. Wellness Coordinator will spend at least 75% time should be spent out on the floor.

Qualifications:
- Licensed Practical Nurse with valid current Pennsylvania license
- Excellent communication skills (verbal and written)
- Supervisory experience a plus
- Knowledgeable of Department of Health Services personal care home regulations
- Experience preferred

Essential Duties
- Daily reports and follows up to make sure resident care issues are addressed
- Reviews the doctor list daily and follows up to make sure resident care issues are addressed
- Wellness Coordinator participates in care planning meetings weekly
- Completes initial and annual PCH Assessments using the correct DHS-required form in a timely manner
- Makes sure changes and updates to the PCH Assessment are coordinated and documented on the medical evaluation and support plan
- Is knowledgeable of resident care outcome trends and issues on the unit
