Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. - Perform patient assessments, evaluations, and follow-up care
- Diagnose and manage acute and chronic conditions
- Collaborate with physicians and care teams on treatment plans
- Educate patients and families on health management

- Current and unrestricted Nurse Practitioner license in Pennsylvania
- A personable, patient-first approach to care
- Strong clinical judgment and communication skills
- Experience in [primary care, urgent care, mental health, etc.] Recommended
