Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Liaison:
Concordia Visiting Nurses is seeking a relationship-driven professional to join our team as a Liaison. This role is ideal for someone who thrives on connecting with healthcare partners, educating the community, and helping patients access high-quality home health services.
This position supports our South Hills and Washington territories and requires regular travel throughout the South Hills region and Washington County.

What You’ll Do as a Liaison

• Build and maintain strong relationships with referral sources, physicians, and community partners
• Serve as a trusted resource on our home health services
• Educate healthcare professionals on patient eligibility and care options
• Identify opportunities to support patient transitions to home-based care
• Collaborate closely with clinical and operational teams
• Represent Concordia Visiting Nurses with professionalism and integrity
