Concordia Hospice of Washington is seeking a compassionate, relationship-driven Registered Nurse (RN) to join our team in a dual Admissions / Liaison role. This position is ideal for a nurse who enjoys both clinical care and building strong partnerships within the healthcare community.
What You’ll Do as a Liaison / Admissions Registered Nurse
Admissions Responsibilities:
• Conduct hospice admissions in the home setting
• Perform comprehensive patient assessments
• Educate patients and families on hospice services and care plans
• Coordinate with the interdisciplinary team to ensure a smooth transition to care
• Deliver compassionate, patient-centered interactions during a critical time
Liaison Responsibilities:
• Develop and maintain relationships with referral sources
• Work closely with partnered hospitals and healthcare facilities
• Facilitate and receive patient referrals
• Serve as a trusted resource on hospice eligibility and services
• Represent Concordia Hospice of Washington with professionalism and integrity
Important:
Must be a Registered Nurse (RN) – required to perform hospice in-home admissions
