Overview
As a Triage Nurse at Concord OB/GYN, the Triage Nurse serves as the main liaison between patients and providers. This is a unique position which allows the Triage Nurse to advocate for patients, while helping providers identify those who require immediate attention and care. This role requires a specialized blend of clinical expertise, critical thinking, and strong communication skills to ensure safe, compassionate, and efficient care for our patients.
Key Responsibilities
- Answer live patient calls and conduct thorough health assessments.
- Evaluate symptoms and medical history to determine the urgency of patient needs.
- Schedule urgent or same-day appointments when appropriate.
- Provide patient education regarding procedures, treatments, and preventative care.
- Triage patient calls and determine next steps (e.g., anticipatory guidance, routine office visit, antenatal evaluation, ER referral).
- Communicate with patients using translation services as needed.
- Collaborate with providers to facilitate timely interventions.
- Serve as a patient liaison to support providers, ensuring timely communication and providing updates or information as requested.
- Interpret laboratory data, inform patients of results, and provide recommendations under provider guidance. This includes, forming and facilitating plan and/or follow-up for abnormal results when necessary, communicating plan to patient, and ensuring follow-up is complete.
- Manage prescription refills as well as requests for 6-month follow-up and mammography orders in compliance with protocols and safety standards.
- Manage prior authorizations for IV infusions and prescribed medications.
- Review incoming medical records, schedule appointments as appropriate, and manage prenatal care transfers (i.e. ensuring complete records).
- Monitor pregnant and postpartum patients (labs, ultrasounds, results, follow-up needs).
- Perform check-ins on prenatal/postpartum patients when indicated for mental health, blood pressure, and any other concerns requiring close attention.
- Analyze ultrasound results reviewing for any changes made to final report from preliminary report and ensuring appropriate follow-up is scheduled.
- Update prenatal care plans as appropriate.
- Respond promptly to patient messages and concerns via phone and patient portal using the same communication method originally initiated by patient.
- Administer medications and/or injections as prescribed.
- Maintain accurate and thorough documentation in the EMR system.
- Adhere to HIPAA, infection control, and regulatory guidelines.
- Serve as a knowledgeable clinical resource for staff, providing guidance, support, and clarification on policies, procedures, and best practices.
Qualifications
- Registered Nurse (RN) license in the state of Massachusetts (required).
- Ability to successfully pass an employment background check and Office of Inspector General (OIG) exclusion check.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum 2 years of clinical nursing experience, preferably in OB/GYN, women’s health, or a related specialty.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Experience with EMR systems; Epic experience a plus.
