Concord Healthcare and Rehabilitation Center, a nursing home facility located in Lakewood, NJ, is seeking a Director of Rehabilitation to join their team.
Responsibilities include but are not limited to:
- Adhere to productivity requirements re: patient care/treatment and management time allocation outlined
- Act as a team leader/mentor to all members of the rehab team.
- Ensure proper identification of patients’ payor source.
- Ensure appropriate authorization for patients with various payor sources.
- Ensure proper HCPCS codes utilization.
- Attend care plan meetings as indicated by facility and regional manager.
- Ensure clinical programs of all disciplines are running smoothly.
- Assist in developing and implementing new programs as needed.
- Ensure screening policies are operating and compliant.
Qualifications:
- COTA , PTA , OT , PT license
- 2 years of Director of Rehabilitation experience
