- Conduct scheduled, in-person meetings with clients in their homes
- Organize medications and assess ongoing medical needs
- Develop medication regimen and checklist
- Dispense correct dosage of each medication at the correct time
- Fill pill boxes or medication dispensers, update with new medication orders
- Reconcile medications and notify clients when refills are needed
- Coordinating with healthcare providers regarding changes in medication, reorder medications as needed and procure refills
- Check labels, expiration dates, and warn patients or caregivers about potential interactions
- Observe and document ongoing changes of condition (remotely viewable by family members)
