- Valid driver's license and current auto insurance
- Current CPR certification
- Social Security card
- Health statement completed within the past 6 months
- Required Florida caregiver training, including Alzheimer's, HIV/AIDS, and medication assistance
- Eligibility for AHCA/FLClearinghouse Level 2 background screening
