Our model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model.

Why is this Role Essential?

The Quality Improvement Project Specialist (QIPS) is responsible for identifying, developing, and executing initiatives that enhance clinical outcomes, operational efficiency, and patient safety across all Concerto markets. This role will report to the Director of Quality & Regulatory Compliance and drives data-informed decision-making through process mapping, analytics, and collaboration with Quality, Regulatory, and Operations teams. The QIPS acts as a change agent - translating insights into measurable improvements that align with Concerto’s strategic goals and regulatory requirements.

This position requires a strong foundation in process improvement methodologies (Lean, Six Sigma, or equivalent), proficiency with data and technology platforms, and the ability to collaborate across clinical, operational, and technical teams to deliver sustainable quality advancements.

  • Facilitates cross-functional project teams — coordinating input from Operations, Clinical, IT, and Quality teams to ensure alignment and progress.
  • Develops project charters, timelines, key performance indicators (KPIs), and progress dashboards.
  • Tracks outcomes of all improvement initiatives, ensuring they are measurable, sustainable, and aligned with organizational goals.
  • Supports post-implementation monitoring to validate sustained performance improvement.

  • Identifies opportunities to automate data collection and reporting processes to improve efficiency.
  • Supports integration of new digital tools and workflows that enhance staff efficiency and compliance monitoring.

Process Improvement and Innovation

  • Maps end-to-end workflows for critical clinical and operational processes (e.g., anemia management, missed treatments, infection tracking).
  • Identifies redundant, manual, or error-prone steps and recommends streamlined alternatives.
  • Develops standardized templates, toolkits, and protocols to reduce variation across regions.
  • Serves as a subject matter expert on process improvement methodologies and teaches basic CQI principles to team members.
  • Partners with IT and Operations to test, pilot, and scale new tools or processes that improve quality and operational outcomes.
  • Continuously monitors national ESRD trends, CMS QIP updates, and best practices to inform Concerto’s innovation strategy.

Collaboration and Communication

  • Works closely with the Director of Quality & Regulatory Compliance and RQRMs to prioritize improvement initiatives and align with corporate objectives.
  • Provides regular progress updates and outcome summaries to executive leadership.
  • Partners with RQS and RCS to ensure improvement projects are supported by accurate data and compliance tracking.
  • Collaborates with CONs to operationalize project findings at the field level.
  • Facilitates stakeholder feedback sessions to evaluate process changes and document lessons learned.

Training and Knowledge Sharing

  • Develops training materials and presentations for improvement initiatives.
  • Supports leadership and staff education on change management, process redesign, and CQI principles.
