Concept Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team in a Skilled Nursing Facility (SNF) in Johnston, Iowa.
We have Full-Time, Part-Time, and PRN opportunities available for licensed Physical Therapists who are passionate about helping older adults regain mobility, independence, and quality of life following illness, injury, surgery, and hospitalization.

Key Responsibilities
- Evaluate patients following physician referral and establish individualized plans of care
- Develop functional treatment goals that promote mobility and independence
- Deliver skilled physical therapy interventions in accordance with evidence-based practice
- Assess gait, balance, strength, endurance, transfers, and functional mobility
- Supervise and collaborate with Physical Therapist Assistants (PTAs)
- Communicate patient progress with physicians, nursing staff, families, and interdisciplinary team members
- Participate in care conferences and discharge planning meetings
- Educate patients and caregivers regarding safety, mobility, and home exercise programs
- Maintain accurate and timely clinical documentation
- Support quality outcomes and patient satisfaction initiatives
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Iowa Physical Therapist license or eligibility for licensure
- Strong communication and interpersonal skills
- Ability to work collaboratively within an interdisciplinary rehabilitation team
- Passion for serving older adults in a Skilled Nursing Facility and post-acute rehabilitation setting
