Concept Rehab is looking for a Physical Therapist (PT) to join our team on a PRN basis in Linden, Michigan.

What You’ll Do

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and independence
• Document patient progress and treatment according to professional standards
• Collaborate with nursing, therapy, and other members of the care team
• Provide compassionate, patient-centered care
  
  

What We’re Looking For

• Current Physical Therapist (PT) license in the state of Michigan
• Graduate of an accredited Physical Therapy program
• Strong communication and organizational skills
• Reliable and compassionate approach to patient care
• Previous experience in a skilled nursing or post-acute setting is helpful, but not required
