What are the responsibilities and job description for the Residential Case Manager position at Concept Health Systems?
Established Miami non-profit behavioral health agency is seeking a Case Manager to provide support service case management to individuals with substance abuse, mental health problems and /or other behavioral health issues.
The candidate for this position is required to have a BA degree in Social Work or other Behavioral Health disciplines from an accredited university and has at least two year's experience in the assessment of client needs, development of support services treatment plan that leads to self-sufficiency.
The Case Manager will be required to possess excellent writing skills to write reports to external agencies that are clear, concise and express in detail the patient’s progress towards meeting the goals of their program. Additionally, the Case Manager must be computer literate and have knowledge of commonly used computer software programs.
Most importantly, the Case Manager must possess an extensive knowledge of support services available in the South Florida community and the desire to provide the highest level of comprehensive case management services to program participants.
Bilingual skills desired – (English/Spanish).
